First Visit
We are conveniently located off Interstate 77, Exit 28 in Jetton Village, Cornelius.
Your consultation appointment will allow us to introduce ourselves and more importantly, meet you and to comprehensively evaluate and discuss your diagnosis and treatment options.
Once your surgical needs have been evaluated and your treatment plan determined and explained to your satisfaction, we will assist you step by step with the completion of your care. This enables us to customize care to your specific needs and satisfaction.
Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip and any X-rays if applicable.
- A list of medications you are presently taking.
- If you have medical or dental insurance, please bring your insurance card(s) with you. This will save time and help us process any claims.

IMPORTANT: All patients under 18 years of age must be accompanied by a parent or guardian at the consultation visit.
A consultation and examination is mandatory for patients undergoing sedation for any type of surgery. Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medications (i.e. blood thinners, aspirin, or have a need for pre-medication prior to your appointment, etc.)
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office; Though there are multiple ways we can get these x-rays, the preferred method is by email: [email protected].
If time is of the essence and the x-rays cannot be emailed or mailed, please stop by the referring office and pickup a copy of the x-rays and bring them with you to your appointment with us.
